STEP 1. FILL OUT ONLINE YOUR DS-11 APPLICATION FOR A NEW PASSPORT

Complete the DS-11 2D Barcoded Application Form online using the Passport Application Wizard

Important: It is required when using a registered passport expeditor to use our company mailing address on the DS-11 application form: In the address field please use: 1318 Coney Island Avenue. Brooklyn NY 11230 in care of Fastport Passport. If you fail to use our address on the application can delay your application form.

TIP: This Online application has replaced “Hand Written” applications, YOU ARE NOT APPLYING ONLINE you are only completing the questions by typing them onto the application. The last page of the application is a print summary page, please scroll to the bottom left corner of this page and CLICK CREATE FORM you will then be able to print out your barcoded application. Please print out 2 copies of pages 5 & 6 only do not sign until instructed to do so. Government fees are paid at the acceptance agency.

Step 1. Recap and Important Information

  1. Complete the form online using the Passport Application Wizard. This will create a 2D barcoded application form all passport agencies now require the use of this form. Once you click on any of these links a new window will open. When form is complete and printed please refer back to this checklist.
  2. Print out 2 copies of pages 5 & 6 of the completed application form
  3. Leave signature blank until instructed to do so by the Acceptance Agent at the post office or county clerk

IMPORTANT: DO NOT SIGN THIS FORM UNTIL INSTRUCTED TO DO SO AT PASSPORT ACCEPTANCE FACILITY IF YOU ARE HAVING TROUBLE WITH THE ONLINE FORM WIZARD AND IT WILL NOT PRINT PLEASE PRINT OUT A HARD COPY APPLICATION FORM

IMPORTANT: YOUR SIGNATURE MUST APPEAR THE SAME ON ALL DOCUMENTS. DO NOT LET ANYONE ELSE EXCEPT THE APPLICANT SIGN THE FORM. PLEASE PRINT APPLICATION FORM ON SEPERATE PAGES NOT DOUBLE SIDED. IF YOUR SIGNATURE HAS CHANGED OVER THE YEARS PLEASE SUBMIT A COPY OF CURRENT SIGNATURE IDENTIFICATION.

STEP 2. GATHER ALL SUPPORTING DOCUMENTS

Print two copies and complete the Passport Processing Authorization Letter. The applicant must check off the boxes as well as write the courier company name (Fastport Passport) on the bottom of the authorization letter. Fill out this form by hand do not leave any fields blank. The applicant must sign the bottom using blue or black ink. If this form is filled out incorrect it will delay the expedited service. Please only check off the first two boxes if you want Fastport to discuss your application with the State Dept.

IMPORTANT: APPLICANTS SIGNATURE MUST APPEAR THE SAME AND MATCH FAVORABLY ON ALL DOCUMENTS

Important: Please DO NOT check off all three boxes on the letter of authorization this may delay processing of your passport

This authorization letter must go inside of the hand carry envelope when you appear at the acceptance agent and the second copy goes on the outside of the hand carry envelope. See step 3 to learn about the acceptance agency and handy carry envelope preparation.

SUBMIT PROOF OF IDENTITY: (Recommended to submit two forms or more of identification)

  • Provide a copy (Front and Back) of applicants valid driver’s license. (Note: Applicants drivers license must be valid for at least 6 months)
  • If applicant does not have a valid drivers license or it has been recently issued you are required to appear at the passport acceptance agent with an identifying witness, who has a drivers license or a valid passport. (A permit or or non-driver id is not considered a valid drivers licence).
  • In addition, it helps to provide copies of at least three Supplemental Identification documents to avoid any delays in processing you application with the U.S. Department of State.

If you are doing a name change it is required you submit your original marriage license, divorce decree, or name change decree.

SUBMIT PASSPORT PHOTOGRAPHS

  • Two identical passport photos (must be 2″x2″) are required for a new passport.

SUBMIT PROOF OF U.S. CITIZENSHIP

Applicant must provide one original document (not a copy) from the following:

Acceptable Documents:

SUBMIT PROOF OF INTERNATIONAL TRAVEL

Applicant must provide two copies of one of the following travel documents below proving your international travel within 14 days of this application submission:

Acceptable Proof of Foreign travel:
– Plane ticket / Hotel Confirmation
– Travel itinerary from a travel agent or online booking (E-Ticket)
– Business letter from your employer. click here for a sample business letter (Note: Business letters are accepted on a case by case basis by the U.S. Department Of State has the right to require confirmed travel plans if they deem the applicants company letter is not acceptable. We always recommend the applicant to have a confirmed travel itinerary or hotel confirmation)

Important: the travel plans must go inside of the hand carry envelope when you appear at the acceptance agent

GOVERNMENT FEES (Applicant should be prepared to pay $195.00 in Government Fees at the acceptance agent

U.S. Department of State Fee – $170.00
Make check or money order payable to U.S.Department of State. -Write your date of birth on the check or money order

Acceptance Agency Fee – $25.00
Leave check or money order blank (Acceptance agent will instruct you to whom to make the check or money order payable to)

IMPORTANT: The U.S. Department of State fee is $170 exact if payment is made incorrectly the passport agency will deny your passport application.
STARTER CHECKS AND 3RD PARTY CHECKS WILL NOT BE ACCEPTED.

STEP 3. SHIP ALL DOCUMENTS TO US FOR PROCESSING

TAKING APPLICATION TO A PASSPORT ACCEPTANCE FACILITY

  • Applicant must Appear in person at a post office, court house, or county clerk and present the original documents to an acceptance agent which is authorized to accept passport applications. The acceptance agent is needed to witness the signing of your application for the passport.
  • Notify the agent you are using an expediting service to “hand carry” your application. Most facilities are open M-F, 9 am to 4 pm.Click here to locate an acceptance facility in your area.
  • The acceptance agent will take All Documents you prepared in Step 1:
  • The agent will instruct you to Sign the DS-11 in their presence. Important your signature must appear the same and match favorably on all documents.
  • The agent will then Seal the DS-11, photos, proof of citizenship, travel documents, authorization letter and the $170.00 check in an envelope marked “To be opened by U.S. Passport Office Only”.
  • IMPORTANT: Make sure the agent returns the sealed hand carried envelope to you. If the agent is not familar with the expediting procedure and is reluctant to release the sealed hand carried envelope, suggest that he/she refer to page 31 of their Passport Agent’s Manual, “Hand-carrying of Executed Applications”.

IMPORTANT: DO NOT OPEN THIS SEALED HAND CARRIED ENVELOPE OR THE APPLICATION WILL BE INVALID.

HAND CARRY ENVELOPE CHECKLIST FOLLOW STEPS: (Important)

  1. Attach your travel plans, and authorization letter to the outside of the hand carry envelope with a staple or paperclip. there should also be a copy of the authorization form and travel plans inside of the hand carry envelope as well.
  2. Attach an additional copy of your barcoded DS-11 application form on the outside of the hand carry enevelope. This does not need to be signed or have any photos attached. This is so the State Department can quickly scan your application in for processing

It is recommended your passport has at least 6 months of passport validity
Your passport must be expired less then 5 years or you can renew early.
If your passport was issued more than 15 years ago, you must apply for a new passport. We can process new passports
If your passport was issued when you were younger that 16 or has been expired for more then 5 years, you must apply for a new passport. We can process this request
If the name you are currently using differs from the name on the expired passport. Submit your original name change documentation. Ex: (Marriage Certificate or Name Change Decree).

PROVIDE PROOF OF IDENTITY FOR PARENTS

You must provide a copy of ONE of the following documents as proof of Identity:

  1. Both parent’s current, valid Driver’s License with Photo (must be issued greater than 6 months ago)
  2. If the above mentioned Identification isn’t available, then you need to supply an Alternate Proof of Identity

  1. Certified U.S. birth certificate (with parents’ names); or
  2. Certified Foreign Birth Certificate (with parents’ names and translation, if necessary); or
  3. Report of Birth Abroad (Form FS-240) (with parents’ names); or
  4. Certification of Birth Abroad (Form DS-1350) (with parents’ names); or
  5. Adoption Decree (with adopting parents’ names); or
  6. Court Order Establishing Custody; or
  7. Court Order Establishing Guardianship.

NOTE: If the parent(s)’/guardian’s name(s) is/are other than that on these documents, evidence of legal name change is required.

Provide Two (2″x2″) Standard Passport Photos.

PROVIDE PROOF OF INTERNATIONAL TRAVEL

Provide two copies of one of the following travel documents below proving your international travel within 14 days of this application submission:

Acceptable Proof of Foreign travel:
– Plane ticket OR Hotel confirmation
– Travel itinerary from a travel agent or online booking (E-Ticket)
– Business letter from your employer. click here for a sample business letter (Note: Business letters are accepted on a case by case basis by the U.S. Department Of State. They may require confirmed travel plans for the child applicant if they deem the applicants company letter is not acceptable. We always recommend the child applicant to have a confirmed travel itinerary or hotel confirmation)

REQUIRED GOVERNMENT FEES

U.S. Department of State Fee – $140.00 Make check or money order payable to U.S.Department of State.
-Write your date of birth on the check or money order

Acceptance Agency Fee – $25.00 Leave check or money order blank (Acceptance agent will instruct you to whom to make the check or money order payable to).

IMPORTANT: STARTER CHECKS AND 3RD PARTY CHECKS WILL NOT BE ACCEPTED.

3. Visit Application Facility

HAND CARRY ENVELOPE PREPARATION

Attach your , travel plans, and authorization letter to the outside of the hand carry envelope with a staple or paperclip. The acceptance agent should also put the travel plans and the authorization letter inside the hand carry envelope as well.

TAKING APPLICATION TO A PASSPORT ACCEPTANCE FACILITY

IMPORTANT: Both parents must appear in person at a post office or court house and present the original documents to an acceptance agent which is authorized to accept passport applications. If both parents are not available, the absentee parent must fill out a consent form DS-3053 which allows the one parent to apply for the passport. This form must be notarized and the notary cannot hand write the expiration date it must be part of their stamp.

The agent will then instruct you to Sign the DS-11 in their presence. Important you signature must match on all documents.

They will also verify the child to the passport photos given.

The agent will then Seal the envelope with the $140.00 check inside, and will then stamp the envelope.

IMPORTANT: Make sure the agent returns the sealed hand carried envelope to you. If the agent does not feel at ease about releasing the envelope, suggest that he/she refer to page 31 of their Passport Agent’s Manual, “Hand-carrying of Executed Applications”.

IMPORTANT: DO NOT OPEN THIS SEALED HAND CARRIED ENVELOPE OR THE APPLICATION WILL BE INVALID

IMPORTANT: YOUR SIGNATURE MUST APPEAR THE SAME ON ALL DOCUMENTS
